Welcome to the HAWWA Wellness Vendor Evaluation Program

We invite your organization to complete this comprehensive evaluation form to be considered as a selected vendor partner for HAWWA's Postpartum Care Ecosystem.

This form enables us to assess your readiness, service quality, compliance, and alignment with HAWWA's care philosophy and operational standards. Upon successful evaluation, vendors will be registered within the HAWWA platform to deliver certified postpartum care services.

Please read carefully before completing this form.

To participate in the HAWWA Wellness Vendor Evaluation Program, your organization must provide complete and accurate details across all sections. This form will be used to assess eligibility, compliance, and readiness for collaboration within HAWWA's Postpartum Care Ecosystem.

How to Complete the Form
  1. Fill out all sections (1–11) clearly and accurately. Incomplete submissions may delay evaluation.
  2. Tick all applicable boxes to indicate your caregiving capabilities, certifications, and services.
  3. Specify your standard hours and price per session in Section 7. Leave blank if not applicable.
  4. Upload all required documents (licenses, certifications, insurance, etc.) in the Documents section.
  5. Review the Terms & Conditions in Section 11, including your organization's readiness to participate in HAWWA's Postpartum Care Training.
  6. Sign and stamp the declaration pages (Sections 7, 10 & 11) confirming authenticity and compliance.
  7. Submit the completed form and attachments to the HAWWA Wellness evaluation team through the designated email or portal.
Important Notes
  • All information provided must be current, verifiable, and supported by valid documents.
  • Vendors found non-compliant with safety, ethical, or training requirements may be disqualified.
  • Successful evaluation will lead to official registration within the HAWWA Vendor Network for postpartum care service delivery.

SECTION 11: TERMS & CONDITIONS

Review and accept the standard terms and conditions for vendor partnership with HAWWA.

A. Standard Work Schedule
Clause Description
Default Working Days per Week All caregivers are expected to operate on a 6-day work week, unless otherwise agreed in writing.
Standard Working Hours per Session Determined by the vendor per service type (as stated in Section 7). A session typically ranges between 6-12 hours depending on package.
Rest & Break Periods Caregivers must receive adequate rest between sessions in compliance with local labor regulations.
Public Holidays Services rendered on official holidays are billed under 'Weekend / Holiday Coverage.'
B. General Service Terms
Clause Condition / Expectation
Professional Conduct Caregivers must maintain courteous, respectful, and culturally sensitive behavior at all times while representing HAWWA-approved services.
Uniform & Identification Vendors shall ensure staff are neatly dressed, identifiable, and adhere to any HAWWA-specified attire or ID requirements.
Confidentiality & Privacy All client information, family details, and service data must remain strictly confidential; unauthorized disclosure will result in termination.
Replacement Policy In case of absence or performance concerns, a replacement caregiver must be provided within 24 hours.
Insurance Coverage Vendors must maintain valid insurance for caregiver liability and occupational health. Copies must be submitted annually.
Health & Safety Compliance All staff must comply with hygiene, infection control, and safety standards during service delivery.
Incident Reporting Any incident involving safety, injury, or misconduct must be reported to HAWWA within 2 hours of occurrence.
Data & System Use Vendors must utilize the HAWWA portal/app for scheduling, attendance, and reporting as part of compliance.
Termination & Blacklisting HAWWA reserves the right to suspend or delist any vendor failing to meet quality or ethical standards.
Audit & Monitoring Vendors agree to cooperate with HAWWA for audits, client feedback reviews, and performance evaluations.
C. Readiness for HAWWA Postpartum Care Training
